Become a Member Physician

Why Join Southwest Medical IPA?

By joining our unified coalition, your independent practice immediately secures professional management of complex insurance networks. We handle the paperwork required for insurance enrollment, coordinate claims and other issues, and maintain preferred provider agreements on your behalf.  Additionally, members gain access to our elite delegated credentialing program, which dramatically lowers the administrative workload for your office staff and helps get new providers paneled quickly.

Eligibility Criteria & Quality Standards

To protect the elite reputation and collective leverage of our association, we maintain a highly selective network. Independent practitioners who wish to become a member physician must meet the following strict foundational requirements:

Absolute Independence

Applicants must operate a truly independent practice within the greater Lubbock area. Our organization receives zero funding from hospitals or outside corporations to ensure complete neutrality.

Comprehensive Clinical Specialties

We accept qualifying providers across primary care medicine, as well as a wide array of specialized diagnostic and surgical specialties.

Rigorous Peer Review

Every applicant must successfully complete our comprehensive, NCQA-approved professional credentialing process. This baseline review ensures that our coalition consistently represents top-tier clinical excellence.

Our Application Process

We have structured our enrollment pipeline to be as straightforward as possible for busy medical offices. To become a member physician, your practice will complete a standardized sequence of administrative reviews:

Request for Application

Our administrative staff will forward a Request For Application form to you to complete and return for the Board of Directors to review. Upon approval your application to join will proceed further.

Credentialing Application Packet

After completion of the Request For Application process above, we will send you an initial credentialing packet.

Submit Verification Materials

You will provide your TSCA (Texas Standardized Credentialing Application) filled out with your education, work history, medical licensing, board certification, DEA registration, proof of malpractice insurance coverage, disclosure questions and signature. We also require physicians to have obtained their medicare enrollment and have an active affiliation/privileges at a Lubbock hospital.

Centralized Peer Review

Our internal Credentialing Committee will execute primary source verifications to ensure full alignment with our rigorous NCQA-approved standards.

Board Approval & Activation

If your application and subsequent credentialing is approved by our physician-led Credentialing Committee and Board of Directors, you will be integrated into our managed care organizations' provider panels with an effective date for their networks and plans.
